We are committed to protecting the personal information
entrusted to us. The purpose of this notice is to let you know
how we treat personal information about current and former
customers.
We do not sell any information about current or former
customers. We do not sell the fact that you are our
customer or customer lists.

Only those who need personal information to provide our products
and services, or as the law requires, may have access to it. We
train our employees on how to handle personal information. They
must follow rules to keep it confidential and safe. We maintain
physical, electronic and procedural safeguards.
INFORMATION WE COLLECT
The information we collect depends on the product or service you
request.
Information from Individuals:
We get most information from your application. We also get it
from other forms you give us. This may include: Name, address,
telephone number, date of birth, social security number, income,
assets, investment preferences, information on health and
habits, information to process transactions or to conduct online
transactions.
Information from Third Parties:
If needed, we may also get information from third parties. This
may include: Person and credit information from consumer
reporting agencies; health history from medical providers and
the Medical Information Bureau, Inc.; employment and benefit
information from employers, plan sponsors and administrations;
replacement information from other insurance companies.
Transaction Information
We keep records of customers’ transactions with us. This may
include: coverage and contract values; payment, loan and

MEDICAL & CONSUMER REPORT INFORMATION
We collect medical and consumer report information to underwrite
life insurance or process claims. We get written consent from
the insured or claimant before we collect it. We do not share
medical or consumer report information for marketing purposes.
We only share it within strict limits as allowed or required by
law.
If you contact another insurer to apply for life or health
insurance or make a claim, the MIB, Inc. will supply that
company with the information it has in its files. A consumer
reporting agency may keep the information it collects. It may
share that information with other companies to the extent
allowed by the Fair Credit Reporting Act.
INFORMATION WE SHARE
We share information only in a limited way to conduct business
and provide service to customers or as required by law.
We may share with other third parties involved in providing or
maintaining your contract or benefit. We share only the
information they need to perform their functions. This may
include: Insurers; Other financial institutions such as
broker-dealers, investment advisers and reinsurers; Your
physician and insurance support organizations such as the MIB,
Inc.; Employers, plan sponsors and administrators. We also
share with law enforcement and governmental and regulatory
agencies where required by law.
ACCESS AND CORRECTION
We want to make sure the information we have is accurate.
Generally, upon written request, we will provide to an
individual the personal information about them in our files. If
they believe that any of our records are incorrect, they may
notify us in writing of the corrections or changes they believe
should be made.
